Rates
$165 per 55 minutes for initial intake session. $145 per 55 minutes for individual or relational session

Insurance
Services may be covered in full or in part by Blue Cross/Blue Shield of Illinois.. Please check your coverage before making an appointment. Call the number on the back of your insurance card to get answers to the following questions:

• Do I have behavioral health benefits?
• Do I have to meet a deductible?
• Is there a co-pay?
• How many sessions per calendar year does my plan cover?
• Is approval required through an Employee Assistance Program?

Payment

Co-payments are due at time of service by check or Zelle. BCBS clients will be billed the balance by Payton Business Services.

Cancellation Policy
To avoid a late cancellation fee you must notify me 24 hours in advance of the scheduled appointment. Failure to do so will result in a fee of $100 to be paid at next scheduled appointment. A late cancellation will not be billed to BC/BC.
